Service Notes & Continuous Check-Ins

The NotesTr Mobile App is used by caregivers and direct care professionals to document key client support visit details required by Office of Developmental Programs (ODP) assisting individuals with Special Needs.

The NotesTr App Check-In Modules are used by organizations to make sure caregivers or direct care professionals are awake for the duration of overnight shifts.

In cases of severe Behavioral Health or Intellectual Disabilities, the Check-In Module also assists with ensuring continuous 24/7 check-ins on clients throughout the day and night.

3 Solutions for High Quality of Care Assurance  & Compliance

Daily Service Notes

Awake Night Checks

24/7 Check-Ins

From a day-to-day oversight perspective, the App and Portal ensure ongoing care assurance and compliance at the Point of Care.

At the completion of a home visit, caregivers log into the easy-to-use App, select their assigned client and then type-in (or dictate) Service Note details summarizing support services provided to including Goal-Outcome recordkeeping.

Caregivers then electronically sign the day's Note. The app automatically logs the date, time and the device's GPS location.

Back at the Home Office, managers or supervisors can immediately view posted Service Notes in real-time.

In terms of operational efficiency, using the app and portal provides significant time and cost-savings.

Use of an App also eliminates the need to go out and collect paper logs and forms, followed by painstakingly tabulating performance outcomes.

Managers using the NotesTr Portal have access to comprehensive reports using filters to obtain performance data that tracks client progress towards meeting goals and outcomes  needed for ongoing compliance reporting requirements and billing.
